If you are unable to give a completed form to the RCIA Coordinator in person, you are invited to mail it, or drop it off at the Parish Office. If you drop it off at the parish office, please be sure to place it in an envelope, seal it, and put Attention Deacon Butch on the outside.
It is not necommended that you email these forms either as they contain sensitive information and a typo in an email address could send them to someone you would rather not have the information.

All forms containing personal information are kept in confidence and stored in a locked file cabinet. All copies are destroyed after you have received the Easter Sacraments, or have left the program.
